Uterine fibroids (leiomyomas) are benign muscular tumors of the uterus that affect 20–80% of women by the time they reach menopause. While many fibroids cause no symptoms, others produce heavy menstrual bleeding, prolonged periods, pelvic pressure and pain, frequent urination, back pain, and — when they affect the uterine cavity — difficulty conceiving. Conventional treatment options range from watchful waiting to hormonal therapy, uterine fibroid embolization (UFE), myomectomy, and hysterectomy. Traditional Chinese Medicine offers an integrative approach that can meaningfully manage fibroid symptoms and, in some cases, support the resolution or stabilization of smaller fibroids.

Important: Large fibroids causing severe symptoms, significant anemia, or fertility complications should be evaluated and managed with a gynecologist. TCM works best as a complement to — not a replacement for — appropriate medical care for significant fibroids.

TCM Understanding of Fibroids

Uterine fibroids in TCM are classified as a form of "Zheng Jia" (癥瘕) — accumulations or masses formed by stagnation of Qi and Blood, often combined with Phlegm-Damp accumulation. The key underlying patterns are:

  • Qi stagnation and Blood stasis — the root of most fibroid formation. Chronic stress, emotional suppression, or hormonal imbalance causes Qi to stagnate and Blood to follow, gradually forming a mass.
  • Phlegm-Damp accumulation — excess dampness in the body combines with Blood stasis to create the dense, fibrous tissue of the fibroid.
  • Kidney deficiency — the underlying constitutional vulnerability that allows stagnation to accumulate and persist.

Treatment Goals

TCM treatment for fibroids focuses on: moving Blood stasis and breaking up accumulation; clearing Phlegm-Damp; tonifying the Kidney and regulating the Liver; and managing symptoms (heavy bleeding, pain, cycle irregularity). Treatment involves regular acupuncture combined with herbal formulas specifically designed to break up Blood stasis and dissolve Phlegm accumulation — classical formulas like Gui Zhi Fu Ling Wan are among the most studied for uterine fibroids.

TCM treatment for fibroids is a long-term commitment — typically 3–6 months of consistent treatment is needed to observe meaningful changes. We recommend ultrasound monitoring every 3–6 months to track fibroid status.
