Edema — abnormal accumulation of fluid in the body's tissues — can affect the legs, ankles, feet, hands, abdomen, or face. It ranges from mildly uncomfortable to severely debilitating and can indicate serious underlying conditions. Always seek medical evaluation for new or worsening edema to rule out cardiovascular, renal, or hepatic causes. Once those are addressed, acupuncture and Chinese herbal medicine offer effective support for functional fluid retention.

TCM View of Fluid Metabolism

In TCM, three organ systems govern the transformation and movement of body fluids: the Lung, Spleen, and Kidney. The Lung regulates the downward movement of water; the Spleen transforms and transports fluids; the Kidney governs the body's "water metabolism" at the deepest level. When any of these systems is impaired, fluids accumulate as edema.

  • Kidney Yang deficiency edema — the most common type. Lower limb swelling that pits on pressure, cold limbs, fatigue, low back weakness, frequent pale urination. The Kidney lacks the Yang energy to transform and excrete fluids.
  • Spleen Qi deficiency edema — pitting edema of the lower legs, worse after prolonged standing or sitting, associated with digestive weakness, fatigue, and loose stools. The Spleen fails to transform and transport fluids.
  • Damp-Heat accumulation — hot, reddish swelling, often in the lower limbs, associated with inflammation. Common in gout-related swelling or infections.

Treatment

Acupuncture for edema uses points that tonify Kidney and Spleen Yang, promote diuresis, and open the fluid metabolism pathways. Moxibustion is frequently applied to warm the Kidney and Spleen and restore their transformative function. Herbal formulas — particularly those in the category of "draining Damp and promoting urination" — are highly effective at resolving chronic edema and are prescribed based on your specific pattern.
